Public MIT repo under vercel-labs, created 2026-09-21, about 13 stars as of 2026-10-23. Uses AI SDK 7 and Vercel AI Gateway with typesafe-ai/jev plus an LLM fallback. Includes editable samples, routing details and optional Resend email delivery. Cloneable with the Vercel deploy button.
Caveats
- Needs AI Gateway access to Jev and gpt-6-luna-fast for live routing
- Sample forms, not a full production helpdesk
- Community forks of the same template are not listed separately
