Claude JEV auto-effort

A MIT Claude Code mod that picks per-request effort from plan step, prompt, and stage, using hosted TypeSafe Jev (TYPESAFE_API_KEY → api.typesafe.ai) as judge with scrubbed payloads and word-list fallback.
